I have had this show up in 2 ways,

first is the server side issue where you crash and have to wait for a few min, then it lets you back in.

the other is i crashed and my eqgame,exe was still running (but i didnt check or reboot for 24 hrs) and i thought i was locked out for 24 hrs. I finally checked and saw eqgame.exe running and forced quit and then i could log back in.

This is pretty accurate from my experience over the past few years.
