Game Mechanics: Hide breaking?

Seems like my hide/sneak breaks pretty often, I will be travelling "hidden/sneaking" past a guard or something and it will just randomly break resulting in a dead rogue.

I'm not performing any action that causes a break, just walking.

Will add SS's soon, it happens very often.

This randomly happened on VZTZ... not too often though.
