Game Mechanics: Missed notes, seriously, wtf?

This wasnt as bad on live :/

I thought it was just my skill lvl at first.. but bard is almost 40, and it still happens way way way too much.

It still happens a ton at level 58. It doesn't seem like there's a significant reduction from the rate of missed notes at level 1 to my level now.
