Only loads black screen, what next?

When I launch the game via Launch Titanium or eqgame.exe with patchme added, I only get a black screen with the EQ cursor showing. Nothing past that ever loads.

Steps I've taken so far to troubleshoot:

Re-loaded P99 zip files into Everquest folder and made sure they overwrote old files
Deleted trouble zone eqg's and txts
Checked eqhost for appropriate server
Re-read directions and retried install, after all else failed.
