People used automated scripts that OCR'd the /list codes to automatically camp the manastone, accumulated numerous manastones, then RMT'd them for BTC pre-$40k price and then road off into the sunset not to be heard from much again except on the forum for lols.

Accounts are probably easy as well. An account suddenly changes its password and is now played by a completely different IP? Seems like that would be a red flag to me although I have no idea what their threshold is for this stuff. I guess if the account had been used as a guild bot that might be harder since it would have tons of IPs in its history.
