The way I remember it, when Kunark first came out, nothing was soul bound. if you died inside sebilis, you had to be rezzed back in or do the quest again. Then sometime during kunark (I want to say it was fairly soon after release), they added the soulbound feature.
I'm almost positive that keys weren't soulbound right away, but thats just from memory.... ...which may not be the most reliable source since i don't remember the teeth mentioned for the ports. I do remember the key needed to go from skyshrine to cobalt scar though. | ||

1. Banked keys stayed in bank after death. I've noticed on this server you don't need to have the key on cursor to open a door as long as it's in your inventory, but I'm not sure if it'd still work if it's in your bank.
2. No idea. My vote would be alphabetical, but that's just a guess. 3. Not a question you asked, but it was mentioned. Soul bound keys were not available at the start of Kunark, in fact, they were introduced much later. | ||

There was no GM support in those locations (or limited support) because they didn't want raiding guilds to make a hack guide to go explore stuff and get info.
I intend on keeping the keys non-soulbound until they were patched and fixed. It adds a lot to the danger element. And that is a key to the risk vs reward for those zones, especially VP. As that key did not become soulbound until much, MUCH later.

I don't remember them being alphabetical. Probably in the order you obtained them? And yeah I think you had to hold them and aim with cursor like a Rogue with lockpicks.
