The epic fist has nothing to do with it, that item actually just hurts a Monk if they can get the Ranger +ATK buff, because it decreases their offhand damage.
It's definitely easiest for a Monk to get some extra benefit out of a Two-hander, because all they have to do is click the weapon after a swing and then click it right back in. The delay of the Two-hander for a Monk doesn't matter with that technique, because they just have to quickly click it on/off. The worry for them is again the epic fist being equipped, because it has a super low delay and when full hasted could cause a primary fist attack to happen if not paying attention to exactly when the Two-hander rounds are happening.

It's a very annoying task to be sure, like working in some kind of ultra fast assembly line where you have to glue your eyeballs to one thing and spam the same repetitive motion over and over, but it undeniably is a big increase in character performance. Not saying it's a great game mechanic, but competitive gamers in other games have to move their hands even faster to win tournaments.

What do you mean about this hotkey? If you unequip a 2hander while auto attack is active you will trigger your offhand punch almost immediately

You have to turn autoattack off after the 2-hander swing, but what I'm saying is you don't have to turn auto attack on + off after equipping the offhand weapon, as you listed in your 10-step writeup. Those 2 steps can be condensed into 1 step of having a hotkey for a manual weapon attack, which helps to keep the whole cycle going without making an error of executing too slowly.
