This would probably be the very best gear:

Ears: Fingerbone Hoop and/or 35/25 JC earring(s)
Neck: Necklace of Superiority
Face: Not really sure, there's not much high-end twink gear here
Head: Blood Ember Helm - might want to get resist gear as well, like Tranix/frog crown

Fingers: 55/5 rings (they're better than Djarn's for a tank, though if you go with fungi, Djarn's might be necessary to offset the -10 agi)
Wrists: Blood Ember Bracers
Arms: Blood Ember Vambraces
Hands: Blood Ember Gauntlets

Shoulders: Crested Spaulders or Tattered Mantle - when the Hole opens, it'll be Imbued Granite Spaulders if your race can wear them
Chest: Fungi or Blood Ember Breastplate
Back: Hierophant's Cloak or Cloak of Flames (SBC might be more realistic)
Waist: If you went with a haste cloak, probably just TBB. If not, RBB (or, more realistically, RBG)
Legs: Blood Ember Greaves
Feet: Blood Ember Boots

Primary: Ebon Mace
Secondary: Atramentous Shield
Range: Idol of the Thorned (also get a RBF for resist set)


Resist set consists of:
JC diamond rings, earrings, mask
Tranix and/or frog crown
Runed Bone Fork
Optional golden wolf's eye bracelets
White Gold Necklace if you didn't get NoS
