I assume because, when you sneak from behind, you can become indifferent to an NPC, and dubious means you will not get attacked when they or a friend turn around.

In Crystal Caverns, for instance, everyone starts Dubious to the dwarves. If you sneak from behind, you can shop with them. Otherwise they will insult you and refuse.
