I don't know if this helps you but... I had this problem, and this was my fix.

Hotkeys have been the same since level 1, and, no, none of my kick/taunt hotkeys overlap.

Anyways, I apparently found the problem. Earlier, I died in Lfay to a random brownie scout, and I didn't exactly know where I died, so I made a /target Apprentice's hotkey to find my corpse, and it was bound to W/S which are also my movement keys. I moved that hotkey off my hotbar, and, apparently, that was causing some weird conflict which didn't allow me to attack anything.

This is 99.9% something wrong or conflicting with your local client, connection or UI settings. Do a fresh install if the other suggestions don't work.

fixed problem by deleting my character files.. very annoying but...

Did they also happen in Pvp Brutality? I can literally stand in front of an AFK caster with attack on and not hit them for 5 - 6 seconds.

I'll try deleting my character files i guess.

I can almost guarantee that this is because of a targetting hotkey you forgot about. Happened to me once.

Couldn't hurt to set eqgame.exe to run in xp compatibility mode, as well.
