Are you using the default UI or a custom UI?

First thing I'd do is backup the INI files in your EQ folder.
Then in game, go to the Options menu > Keys and press Reload Default.

If that doesn't work, it could be another program (like Media Control) interfering with EQ. Try disabling programs you're not really using.
