Any epic MQ or whatever gets you closer to an epic is worth it. Most game changing epics IMO in order of best that are actually somewhat obtainable with a limited budget:

SK
Shaman
Monk
Rogue
Cleric
Druid

Really the only thing that is actually worth MQing in the Monk epic is the pipes since those are actually super annoying to get. You can skip the other annoying stuff by just killing Brother Q with a single group in SK.

Immortals drops all the time in Skyfire, so you can easily get it just by hanging out while Necros and others are soloing there.

And then the other kills are so simple, with the exception of Xenovorash where you need to get like two groups together instead of 1.

-Two or Three Tinkerer's Bags for ~10k-15k can go a long way to never being overweight in a dungeon.

And can instantly resell to a Merchant for a tiny loss, or sell to a player for a tiny profit.

I always tell people if they have 5+k in their bank that they don't immediately need, spend it on a Tink bag.

It depends on relation to what other cheaper items can do.

I wouldn't describe the difference between a Cloak of Flames and Silver Chitin Handwraps to be a "gamechanger" even if it's a strong upgrade. You probably wouldn't even notice the difference if no one told you they swapped the two.

Several items are too cheap to qualify since they fall below 10k in price, e.g. Jboots, cheap haste options, Solist's Icy wand, Singing Steel Helm, Deepwater Helm and other Kunark armor clickies.

Other items are too expensive to qualify, e.g. Fungi Tunic.

Some mentions from me are therefore,

JBB: Changes how you play a Shaman, at least during part of the leveling experience.
Fungi Staff: Similar to above.
CoS: Clicky invis is a bit of a novelty or niche thing but still think it qualifies
Lumi Staff: Similar to JBB
Ikky BP: If not too cheap to qualify still cuts downtime significantly for several classes.
Burnt Wood Staff: May also be too cheap to qualify but free nukes are great!
Blood Ember Greaves: Extra manafree FD. Huge!
Cobalt Bracer: May be too expensive for this list but shrink is BIG (heh) for big races.
Cobalt BP and Singing Steel BP: Invigor is nice if you can be arsed to click it!
Deepwater BP: Huge for paladins who got big bags full of jaspers!

I must have missed many many items but those are some that I can think of.

Not sure how much plat it would go for these days, but loot rights to either one of the Soul Well staves from VP can be game changing for a Necro.

Rend Robe loots rights are similarly game changing for a Wizard.
