Out of curiosity, what are the other 3 times?

I can only remember one other suspension against you in the last year, but I could be wrong.

Agatha was suspended for allegedly training azrael. Came to the forums and threw a bitch fit. Derubael came in and said he was going to review the case until he saw the thread. Agatha still suspended. Thread derailed.

Wait I see another one but it has nothing to do with Azrael. And I don't see a log of the one I remember, so that may have gotten overturned.
