I noticed the full screen issue myself when I started playing recently. I discovered that what's happening when you're in full screen is that the clicks you make are making it all the way to the desktop. So, for example, when I clicked on the EQ button in the lower left corner to bring up the menu in game, it minimized EQ and launched Internet Explorer, since the IE pinned task bar icon was directly behind the EQ button in-game. The IE pinned icon somehow received the click event, instead of (or in addition to) the game window. This happened for any icon that could receive click events on the desktop.
I kinda suspect this is a problem specific to Win Vista and forward. The OS probably changed the way window z-index and/or event propagation works since EQ Titan was created, and as a result it changes EQ Titan's behavior. I'm not sure there'd be an easy solution for this, so I ended up just learning to play in windowed mode (which, for some reason, doesn't have this problem).

You can't alt tab without it going into windowed mode, and going back to full screen WILL crash the client eventually, so i recommend staying in windowed mode. But there are a few things you can do to make it at least work somewhat properly.
Check your options and make sure its running in the same resolution in windowed that you will want it running in full screen. If they are at different resolutions it will cause the mouse to either be restricted to a small area that = the window resolution, or it will just minimize the game when you click out of that area. Then once you have launched the game alt+enter into windowed mode, and move the window around a little bit, then put it back into place and go back to full screen. Now you should be able to Alt+Enter back into fullscreen and have no mouse issues. But like i said the client will crash on you eventually, its EQ.
