Did you give it to the mob before you charmed it? If so then that's the reason.

Oh Frag I missed this. No the mob was charmed. The last time it happened was in KC basement and had https://wiki.project1999.com/Skeletal_Caretaker charmed.

If you just walked up to a friendly NPC and handed them a Torch would they just equip it? I didn't think that was a thing. I thought pets had a special equip mechanic.
