Try checking the Xpos and Ypos values in the [PlayerWindow] section of the UI_Charname_project1999.ini file in your everquest directory, where "Charname" is the name of the character.

You could copy the values from a different char's UI file. If that doesn't work, you could even backup the original and copy a different char's entire UI file over it.

use the EQ menu button, you can fiddle with that to turn on and off different UI on your screen.

You need to make sure your windowed resolution is the same as your fullscreen resolution in the eqclient.ini file. I presume when you switch from fullscreen to windowed it throws your health bar box off the screen.
