Have to be careful using Allakhazam as a source being that most of the data is post Velious. Some of it is close (2002).

Maybe it would be a good idea to search and see if there were any changes in the patch notes to spell drops from 2001-2003, etc

Vendor spells appear to be from (Xorbinasticalus Zimralicus, gnome vendor in Captian Nalot’s hut, +4631, +1320, Iceclad Ocean and Loremaster Dorinin, Thurgadin A Cleric guild), unless otherwise listed.

Evidence that dropped spells and vendor spells are similar to runes in that mobs drops the spells related to level, see note from Turning of the Unnatural.

from what i remember all dragons dropped lvl 55+ spell and if I remember right they would drop at least 2 on every kill + a chance at some other drops. This is from memory, but I remember duoing dragons to get the spells to sell. Kael drakel arena had a chance to drop 55+ spells with maybe < 10% chance

Vandy has created a google doc for helping to track which npcs should drop which spells. Posting this here for more attention, and updating OP with it.

Velious Spells Looking for people to help participate in research for Velious Spell drops. I have a google docs spreadsheet with the information I have been collecting that anyone can view. There is a comments section and if you are logged in you can real time chat on the side. People can be added as contributors to the project or post information here to be updated.

Velious Spell Sheet

1. We need to get an accurate list of which NPCs dropped which spells.
-i.e I find evidence Composition of Ervaj only dropping from Lodizal and Woushi using wayback machine Alla , Casterrealm, Graffe

2. We need to know how many spells each Raid Boss could drop and what % chance to drop that group of spells
-i.e Lodizal would drop 2 spells, had a chance to drop a 3rd spell and even less chance to drop a 4th spell. ( 2x100%, 1x50%, 1x20% ? )

3. What spells were changed and how through the Velious timeline. There is a 2nd sheet for this. The first sheet will deal with spells that were in at Velious Launch.

Aegolism definately dropped from lodizal, i rememberd getting more than 1 on a few occasions.

Logs from Mac server. Always dropped 4 spells. I had more logs, but many of them I would just ignore the spells since they dropped so often (12hr respawn x 4 spells etc.). But I never remember a kill where he did NOT drop 4 spells. All 60 velious spells should be on his loot table.
