I have maybe ran into this a hand full of times. I definitely wouldn't say it's a lot. Most people are appreciative. There are always the exceptions. I usually chalk it up to being really high on drugs, preoccupied with kids/work irl, or just being a narcissistic son of a bitch. But you should always say thank you to someone doing you a favor, whether they went out of their way or not.

It depends on who you talking to regarding the overall attitude of the players on p99. Some people will say "no one appreciates anything," "no one ever helps me," "no one says thank you EVER," etc. These typically are people that always see things from a negative perspective.
Personally I agree with everything you just said about why SOMETIMES people don't say "thank you," don't say "please," etc. I always remind myself that all of us are living IRL as well as interacting in the game and sometimes the amount of input from both can be overwhelming and stuff gets missed.
There are the rare ones that honestly think they are above issuing any sort of recognition of others in the game but I really believe this to be a very small minority.
