Here is the problem: it doesn't matter how much hp/ac you have if the enchanter is tanking. The simple truth is that there is no good solution until VP/epic. About the best you can do is make friends with a shaman or two. A good shaman will make even a poorly geared tank look pretty good (+800HP for CH, slow on tough mobs, +50 dex for procs, +50 str for extra damage, and so on).
Anyway the OP is in fact a shaman himself (I'm guessing a barbarian with the Spiritist's hammer). Personally I think its a bit overrated - at 50+ and especially at 56+ your best bet is root rot a ton of stuff with the epic dot and a few plagues thrown in here and there. But I can see the appeal to the battleshaman. Sometimes I've considered making a halfling 'paladin' (cleric)
