The best one I remember seeing way back when EQ was new:

Shaggy has defeated Scooby in a duel to the death! Scooby has fled like a cowardly dog!

Another one I saw:

Dominos has defeated Pizzahut in a duel to the death!

And one I was responsible for:

(Anybody) has defeated Hitmeplease in a duel to the death!

Slightly off topic, but another funny name event was back when Master Yael in the Hole would death touch and call out names to the zone. I made a halfling rogue and just for kicks said "Hi" to Master Yael:

Master Yael shouts,"Imyodaddy!"

Ah, the good old days. hehe.
