Make sure your video drivers are up to date and and as Mcbard mentioned make sure your resolution is set right. What OS are you on? Windows 7? Windows Vista? XP? Make sure EQ is in C:\Everquest and NOT in program files. If all else fails, reinstall and use the all in one installer in my signature, it will set it up all for you properly and let you choose what resolution to play at. I would also try to run it in Windowed mode to see if it runs.
