Random drop from trash in Karnors and was removed from the loot tables shortly after RoK release IIRC. Was also a ring with like +10 str that dropped in the area, cant recall the name but it wasn't around for long eaither

Gem Encrusted Ring [AC10 +7str] - I had one - dropped early in Kunark off random giants in Dreadlands & Frontier Mountains (I got mine in Dreadlands, not far from Karnor). Was removed from their loot table after a month or so I believe.
