I played in 1999 as a Paladin, and I remember it refreshing upon death.

I can't speak for LoH, but Harm Touch certainly refreshes upon death. In fact, when I played in the summer of '08 on the Combine, it was still refreshing upon death. I took advantage of this a few times.

If any exploits are worried maybe just reset the timer offset to 5 minutes?
