Juuuuust cause I'm OCD and you never know what you will find, I went with a fine tooth comb over the outstanding spell lists. I'm glad I did as there are a few interesting finds (will explain those at end of post)! Here is 58 more from your list with proper era tags:

Here are the last outstanding ones, I suspect they are classic, but I couldn't find specific mob information. The Maestro Lifetap seemed obvious, but the ID made me think it was an upgraded version in the client. I dunno, here are the ones I'm too lazy to get sure on and am giving up on:

Notes on findings from the first list:

Blast of Cold:
I can't pin point it, but I believe with high certainty this is an out of era lower level wizard spell which replaced Shock of Frost, I can find no mention of it in era. P99 currently has an item (Ice Crystal Staff) that procs this and it is incorrect. Confirmation from my log below:

[Sun Apr 16 20:49:45 2017] You punch a goblin scryer for 33 points of damage.
[Sun Apr 16 20:49:45 2017] You resist the Blast of Cold spell!

The Ice Crystal Staff should have 'Shock of Frost' proc on it, not 'Blast of Cold'. Evidence:

Kind of odd how the ID of '372 Blast of Cold' on the spell appears classic when you compare to the other IDs, but '8447 Shock of Frost' appears to be sequenced much later.

Gangrenous Touch of Zum`uul:
This appears to be a Level 60 Necro spell that would be in the scope of the server (assuming you are going to revamp Chardok):

This post seems to indicate this spell should drop off a rare mob after the Chardok Revamp (Grand Advisor Zum`uul). Text from comments:

Me and my guild camp royalty for few hours other day killed king 5 times and queen 4 times but in that time a rare spawn called grand advisor zum'uul spawned and dropped a lvl 60 necro spell called ganrenous touch of zum'uul and i was lucky enough to win roll asked many high lvl necro's if they had and many had said no so wondered if any others have seen this mob..

Granted that comment is from 12/14/2001, it is very close to era. I don't see any patch notes since chardok revamp and this date that indicate any Necromancer spells were added. Only thing that comes close is:

- Removed some bad NPCs in Chardok. (12/12/2001 patch)

I don't see how that would relate to that spell. Not sure if other NPCs at this time (level 60 necro NPCs) could cast this? Maybe this spell is on your radar for the Chardok revamp, but maybe it slipped through the cracks. Either way, I bet some necro's wouldn't mind a Disease Based Lifetap (assuming on PvP server it would be great?)

Looks to be added with Hate revamp (Lord of Ire should cast this, or proc not sure. Not sure if currently doing on P99).
