You have to "know a guy" and be well-connected to get a "good opinion" or a good surgery or to increase your chances of a great result in the US. Or get a good treatment for anything really.

Care varies wildly depending on the doctor, even in a 1st world country. Social networking with successful people is priceless in any country.

Same goes for mental health, I know a lot of upper-class guys who were in high stress jobs and divorce situations who only pulled through because they knew a wealthy guy who knew a well-off guy that helped so-and-so. Word of mouth referral. Saw other guys with no networking get treated like shit by bad therapists and doctors who didn't care about patient #2359034 and not get better and their career tanked.

But even if you aren't networked, your chances of getting paralyzed from surgery or a hack job are significantly reduced in the US. That's the benefit of 1st world education.
