Lieutenant Milven was added with the Nov 2002 SS revamp as well.

The quests for Giant Toes as well as Giant Warrior Helms should be on the two entrance sentries from WL side.

I believe they took Storm Giant Toes and Hill Giant Toes(bug here not sure when it was fixed to Frost Giant Toes).

This is the way it is currently working now, and should be tested if it hasn't been. Kcor gives quests for helms, mercs, and toes. Enots gives quest for helms.

I do not have hill giant toes as a possible quest component.
