Live was a total cluster at the top end too, at least on my server. Spawns were dominated by a top guild, with a couple of others nipping at their heels and being blocked on critical bosses continually. Not saying that's the ideal, but it was classic EQ.

Yes, but on Live if a GM was sitting in a zone and watched a guild exploit for hours they wouldn't have went to the forums and made a post saying "stahp it guiz srsly" and then let them walk.
TMO should be banned in its entirety. (Whoever was present at that specific time alone, nevermind the multitude of other offenses that warrant bans that the rest of us would have received if caught in the same manner.)
Until the corruption is reversed all this talk of "Fixing" shit is moot. TMO has been allowed to fester and gather loot and recruits they would otherwise never have if the staff did their job properly. Sirken is just perpetuating the problem with his bullshit excuses of timeframes of offenses.
Enforce existing rulesets before inventing new ones.
