Waaay back in the day...
<Remember when hitting the 'A' key would enable auto attack?> As a new player, you target a guard (or merchant) and start typing to talk to him... "Where ca"... Guard soandso hits you for 120 points of damage.. You have died.. Loading Please Wait.. Happened to me a few times until I learned to remap the auto attack to the "~"
