Patient denies pain. Reports feeling "fine." Flat affect. Denies suicidal thoughts or behavior, homicidal thoughts or behavior, and self-mutilative thoughts or behavior. Thought content and processes reality-based and appropriate to situation. Current major stressors: "Medsurg exam." Level of anxiety: Mild. Current level of coping: Fair.
