Even when they launched Sullon Zek, the penal colony of EQ whose sole purpose was to isolate the most belligerent and savage players, it was sans itemloot. It was known already by then that itemloot didn't work so well in a game where it can take months to obtain an item and where some classes are completely helpless on their own.

I'd say that a hypothetical new PvP server would need teams as well. Without teams, it basically just becomes a guildwars server that only really works for those who are in one of the big guilds. A teams ruleset will give players some breathing space as there will be places where they can relax and not worry about getting ganked over and over. It also adds an element of politics to the server, something that's missing when the entire world is just a non-stop warzone which doesn't suit EQ's design.

The teams shouldn't be based on class or race, though. That leads to unbalanced teams and weird PvE. I'm inclined to say that there should simply be Team A, Team B and Team C, and you choose which to join upon creating a character. Give each starting character three books, and by level 6 or whenever PvP becomes enabled, they must turn one in to the Priest of Discord and choose a team in order to continue gaining XP.
Great post! MOST players do not want item loot. I always appreciated no item loot when I played on VZ and SZ. You explore more, and fight more.

Team servers were the best! Vallon Zek turned into guild wars because of all the cross teaming. I’m sure Tallon Zek was the same way. I wish they had locked teams like Sullon Zek. I had the most fun on that server. Why couldn’t they use the same format? I thought all 3 teams (evil, good, neutral) held their own raiding and fighting. I didn’t notice a big imbalance. I think Evil just had the biggest pop.
