NPC Spawn Issue: Lguk - mobs missing / wrong place

Just had a small trip of Lguk on R99.

In frenzy there were no bloodthirsties, instead there were 2 live frogs and an undead one. As I was looking at these strangely placed mobs, one poofed into thin air.

There was a Minotaur at lord camp. Several camps had no mobs, despite not showing any indication of having been cleared.

I felt uncomfortable exploring more.

Have some pics but don't feel they add value.
