My suggestion is to start over doing this:

Install a fresh Titanium install. Before doing anything make a copy of the entire directory and set it aside in case you have to start over.

Then go through the steps in the "getting started" thread rather than using the patcher....there is a link on the main page on the left menu bar. In that thread there will be a link to the current spell files and will give specific instructions on what to do for each step. Just follow the instructions as listed in order.

Also in that thread there will be a link for WinEQ2. Download and install that. Launch it and configure for p99. If memory serves, all you have to do is point it to the EQ directory. Then have winew2 generate a desktop launch icon. use this for launching p99 not the normal one.

That should fix your problem. A lot of newer systems conflict with this older software but wineq seems to fix it. You will have to play in a window but you can maximize it so you hardly notice. And its handy to be able to tab over to a browser page with the wiki and forums up while playing.

If you get this far and everything is running, you can copy over your old inis and UI files if you still have them.

For whatever reasons I was able to play fullscreen using just the client a few years ago but have had to use the wineq workaround for about the last 2. Haven't had a single problem since, though.
