Game Mechanics: Character not swinging, creature not taking damages

I am currently on my shaman Emeko in Lake of Ill Omen hunting, trying to get my level. This has occurred several times today. My character, anytime during combat, will begin to stand there, doing nothing. At the same time, the damage from my DoT, Affliction, is saying it's dealing damage, but the creatures health bar is not reflecting this. Unsure if it's lag, as I've logged several times, even restarted my PC several times. It's becoming frustrating and almost unplayable.

Always happened with one mob, but there was a case where I got an add and still nothing happened. It may have been the zone, or myself, but I later went to SK when my group got on and didn't experience any issues.

Ive had this happen fighting brutes in WW once. had one infront of me targetted and had attack on and nothing was happening to its health. Spellcasting nothing changed it and it just kept beating on me. Had to root it then camp out
