Pharm does measure the adverse effects and especially deaths during clinical trials on everything.

No COVID deaths among either the vaccinated or the control group were observed during Pfizer's clinical trial (ctrl-f death if you are lazy). It should be obvious that if no one died, the trial did not prove that the vaccine reduces death rate.
