Crashing Reset Device Failed

Only zone I crash in is OT. It's random and can happen consecutive times in a row.

Reset device fails pops up and game crashed.

Running WinEQ2, SP2, run as admin etc.

Can't seem to come up with any solutions.

Any help appreciated
