I believe the Blood stained note shows up as "A canine" in the database as he treated it as such. I did the quest a long time ago and did not require any specific faction level and my faction level is amiable. I finally got the note to drop and when i turned it in, he replied with the " he holds the canine in his hand..." which is the incorrect response for the quest.

The reward item was not returned.
