Default Dual Wield & Double Attack Chance by Skill Level

Anyone have a link to a spreadsheet or know offhand what the chance is to dual wield at each (or just max) skill level? Is the chance to double attack with primary hand the same as the chance to dual wield? What about secondary hand? I know secondary hand doesn't begin to double attack at all until the character reaches 150 skill.

Dual wield chance:

Monks
(level+skill)/400 X 100 = % (78% at level 60)

wars/rangers (maybe rogues?)
(level+skill)/500 x 100 = % (60% at level 60)

Fairly certain that offhand has the same double attack chance as mainhand once you hit 150 skill, though obviously a double attack will only fire when your offhand swings.

Also,

1. are the double attack chance formulae the same as the dual wield formulae you listed above?
2. Does anyone have any idea when the DW check is performed? Is it performed on every main hand swing, or every time the offhand delay cycles. In other words is it possible to swing with your offhand without your main hand? I suppose watching my swings could tell me this if I equipped two different weapons ^^

1. Double attack is different. Some have said this is the formula, but I don't see any truly clear resources on this: % chance to double attack = (your skill) / [("max skill")*1.05], where max skill is supposedly a constant with the value of 400. This would give most classes a 60% double attack chance.

2. Dual wield checks are performed at the delay of the offhand weapon. Mainhand and offhand are completely independent and can swing independently. In other words the answer to your question is yes. One interesting note here is that offhand can proc even if the dual wield check fails, so the "proc check" happens before the DW check. This makes something like venomous axe a really nice offhand at low levels when you rarely swing offhand.

Another thing worth mentioning is your off hand weapon can proc even on a round when duel wield fails to make it swing.

Another thing about duel wield is swinging a two handed weapon doesn't reset the off hand timer, so you can unequip a 2 hander between primary swings to throw punches with your off hand.
