Lambent bracers quest - anyone else have this problem?

When I gave the items to Orstorm to get the lambent fire opal I got an error to the effect of "Server found no item in slot x (-> y), deleting item!" and all of my items got eaten, with no quest reward. I assure you I did absolutely everything right. Has anyone else encountered this issue?
