so, since the server is down ATM im going to ask this here.
lets say i used the one charge in the Skinned Halfling Face Mask for funsies.

Do I go back to nektulos and kill master whoopal again for his head?

Do I go back to the dude i got the quest from and see if he'll give me a new mask?

The former. It's a regular quest turn-in, so you get the reward for turning in the head.
