Default Monk Feign Death Fail Rate!

This issue has been brought up a few times before, but I would like to cover this myself with a little more detail and info.

I played an iksar monk from Kunark release and got him to lvl 60 before Velious on live. From what I can remember myself, FD fail rates after 100 skill where far and few between. There have been many posts over the years on Project 1999 about FD seemingly being off esecially at lower lvls, and even still at max lvl, just not as much. Ive lvled my monk to 24 on P99 and got his skill to 125 ( max for the lvl ). My FD fails a TON, nearly half the time I would say. This is definatally not how it's supposed to be. This is what I have found so far:

Patch notes from 2004:

------------------------------
December 15, 2004
------------------------------

- The Monk's feign death success rate has been improved. It was
previously capped at 95% at 100 skill and now it ramps up the success
rate for higher skill levels.

What I can gather from this, is that prior to this patch ( presumably from release till this patch ), once you had 100 skill in feign death you had a 95% success rate, no matter what your lvl ( conicidentally this is the exact same cap ( 95% ) that casters have for trivial spells, which makes sense.)

On P99 there seems to be some sort of lvl based increase added into this formula, ignoring some, or part of the skill level. I tested out FD on my lvl 24 Iksar monk with 125 FD skill 20 attempts. I had 10 fails. This is in no way a scientific parse, but note I had 5 FD failures in a row at one point. The percent chance of failing 5 indipendant FD attempts in a row at a 5% fail rate is 0.0000003125% .. or 1 in 30 million or so... There is something wrong here...
