There are some serious issues with /duel and /decline

I'm not even sure I know where to begin. I can't even remember the last time /decline actually worked - meaning, the last time I typed /decline and it said I declined a player's duel, and then I was actually able to initiate/accept another duel without having to camp out and back in first.

Does anyone know the specifics of this problem? Seems like it would be a simple fix.
