Default Unable to Attack

Frequently during fights, my character (Monk, 45) is unable to attack. Attack will be turned on, and I am in melee range, but my character does not swing at the mob. I am using a magical weapon (Imbued Fighters Staff). It happens regardless of weapon type (tested with 2h staff, fists, and throwing stars). It often happens right in the middle of attacking; my character will just stop swinging as if he took a vow of nonviolence.

In City of Mist tonight, it was happening literally every single fight, and sometimes multiple times per fight. This went on for an hour and a half before I hopped offline.

I've experienced this in three zones so far: Rathe Mountains, Lower Guk, and City of Mist. I can't confirm whether it occurs elsewhere. I have talked to at least one other person who sometimes has this problem.

I also encountered this problem months ago in Highpass Keep. I didn't encounter it again until now. It's been happening since Sunday afternoon (4/27/2014). I played all day Saturday without experiencing the issue.

The only way I've found of fixing this bug is by clearing my target (escape), and then regaining the target. I often have to do this several times to fix the problem. I'm hoping that there is a permanent fix. As you can imagine, it is not only immensely annoying, but can also put my party in bad situations if it starts really acting up at the wrong time.

For reference, the only client-side modifications I have made are a custom UI (DuxaUI) and using a few luclin models. When I encountered this bug months ago, I was not using the Luclin models, nor did this bug suddenly pop up when I started using them. I used DuxaUI back then, but I used Duxa for months without having this problem happen.

Update: I've tested, and this bug is only happening on my monk. I wasn't able to replicate it on two other characters, though I was able to replicate it tonight on my monk in another zone, Emerald Jungle. I tried all three characters in succession, and thus confirmed this has nothing to do with latency. This time, my monk just stopped attacking as soon as the mob started running. At 3%, no amount of turning attack on and off and reacquiring target would fix the bug, so I was unable to kill my target. This bug has officially gotten to the point of preventing me from playing.

Update 2: Alright, I think I've fixed it. The fact that it was only happening on one character made me suspicious, so I did a search on my EQ directory for every file with my monk's name. I deleted them all. Now, I have to redo my UI, but this problem doesn't seem to be happening anymore. Score. Also, looks like macros are stored client side. Didn't know that, but it makes sense.

This happened to me in the sarnak fort yesterday. Also a monk.

Clearing target did nothing, had to let the mob reset. What i noticed was that the mob appeared to "warp" back to its spawn point, rather than pathing normally. It happened when I pulled the mob up the ledge, but not if I fought it at its spawn point.

For me it happens regardless of where the mob is, and several times a fight, every fight. I did not have a single encounter yesterday where this bug failed to occur.

old fix for this used to be target your self and then target the mob again, but would be nice to get a perma fix of it

Right. I made an assist macro which targets myself before assisting the tank, but I noticed this doesn't work as well as using escape to clear target first.
