It's a self depricating phrase.
A defense mechanism, so we can all cope with the fact that we still play this ancient nerd game well into adulthood.

early blue server days everyone was an elf for whatever reason, mostly female because horny and eq was back. 5 years of kunark and 3 or 4 years of velious did weird shit to people
on green you see more human/dwarf/erudite casters, more ogre everything and even more iksars because we know better human casters because stamina or strength is more in demand because int in high end gear is easy as fuck to come by and even a half elf druid with their lowass starting wis can get to 255 in some tov gear ogre everything because frontal stun is just that strong and with a shorter vanilla>kunark timeframe people just waited out for iksar necromancers

Because EQ is an Elf Simulator.

People may refer to EverQuest as "Elf" because the game features a race of elves as one of its playable character options. The elves in EverQuest are known for their grace and agility and have a distinct appearance with pointed ears and a slender build. Therefore, the term "Elf" may be used as a bitlife shorthand way of referring to both the race in the game and the game itself.

"elf sim" is whatever, i'll never be able to look at "Fay Gate" the same way again due to red players.
tl;dr this place used to have a community, a 4chan lite community but a community none the less. sorry you don't got.
