Default Feign Death not indicating it's failed randomly

I just died because my deign death didn't say that I feel to the ground, I figured I was safe but I look over and I've died and lost experience.. [You must be logged in to view images. Log in or Register.]

Is this how it works? I've almost always seen a "Collie has fallen to the ground" when my FD fails.

I found this sometimes happens when a mob hits you at specific moment when you are falling.. usually when there is a pack of mobs beating on you. No fail message (or spell cast) , just mobs still beating you.
