the velious era plane of hate revamp just unlocks the 2nd floor, moves innoruuk to a building up there with a different loot table with racial items, and adds the minibosses with innoruuk's current loot table; shield of the immaculate is a rare drop from lord of ire at that point

eerie chest doesn't exist until the may 2003 revamp that actually changed the zone layout and changed innoruuk to a level 65 raid mob while moving his racial drops to trash mobs
