I've KS'd before too just by doing random fly bys at GM Speed in ecommons doing my usual circle running. I sometimes target a mob and try to time my attack key to hit it, and since I'm running to fast it hasn't told me about the player sitting next to said mob trying to kill it.

And another funny thing about it is sometimes I'm GM hidden, so the player never sees me, all they see is "a large rattlesnake has been slain by pain and suffering." and they get no XP and corpse locks for 2 minutes.
