Now to try and put it in newbie terms, Simply put most of these proxy programs you are running support a simple TCP connection where almost every standalone game like EQ, WoW, D3 and such do majority of connections via UDP. There ARE certain socks/5 proxys that support tcp/udp but nothing you are going to find free. I don't know if elaborating more is appropriate because last thing we need are a bunch of idiots learning how to run proxys to box p99. Plus the last thing I need is to get forum banned because I am try to explain to some apes/trolls the workings of certain game connection protocols.

They are all over the world, A large portion of them usually near data centers in very populated areas where there is a heavy bandwidth demand. That includes places like Chicago, Phoenix, Los Angeles, Miami, NY, ATL etc

only way to control would be to end ip exemptions and probably ban all accounts from high VPN/IP proxy countries
