Well, to add to Ele's evidence of Deep needing to be killed in order for Xenevorash to spawn.

Note: It is safest to have about 3 or 4 groups so you can be assured that you will win this fight. Once you hand the Eye of Kaiaren to the Astral Projection in Lake of Ill Omen, Vorash and Deep will then spawn. Deep and Vorash both hit for 170 and proc a DD and Stun. Note: Have one group on Deep while the other people are all on Vorash. Once both Deep and Vorash are dead (they drop no items) you then must wait a couple minutes for Xenevorash to spawn on the platform.
