Considering I don't carry any of the keys on me, going down to the next level where it spawned could be hard. If it came from there it probably came up through the floor (or the adjoining hallway opposite that caved in side) or something aggroing on you :x
The only thing I can think of is that you aggrod the placeholder itself when it spawned and you died before I could finish my nuke. I usually rez those poor bastards that the PH (or Gynok) owns tho.
