-This usually happens when you alt-tab frequently, run another 3d program simultaneously, drag EverQuest window to a separate monitor, etc..

-It can also happen if you do not have a working version of DirectX installed (or something has been corrupted).

=A bad memory module could potentially (though unlikely) be the cause.

-Net Framework 1.1 and 2 installed?

-If you're using Windows Vista or 7, change compatibility mode to Windows XP.

-Might be resolution related issue. Easiest method here is to run the OptionsEditor.exe and select a low resolution say 800X600 that you know your monitor supports.
Try a few just to be sure. If you can get into the game, load up a character then immediately bring up the options window and adjust your display both fullscreen and windowed 'video modes' to something that your monitor can support and is best for you. That should save it from there on.

-And as a generic consideration, try running it as administrator.
